Announcement - Northridge Local Schools Partners with FinalForms

We are very excited to announce that the Northridge Local Schools has partnered with FinalForms, an online forms and data management service that will be replacing paper forms. FinalForms allows you to complete and sign enrollment, back-to-school and athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. 

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of students use FinalForms and complete their portion ASAP for the 2020-2021 school year.   

IMPORTANT: For current students, you will receive an account confirmation email from FinalForms if we have a parent e-mail on file.  Please click the link and follow the steps to login and access your student(s). If you have not received an email from Final Forms within 24 hours please check your spam folder or contact the main office at (937)275-7469 to submit your parent e-mail address.  If you are a parent of a middle school or high school athlete, please log on and fill out the forms with your existing username and password. 

Note: If your student is NEW to the district (kindergarteners and transfers), please be sure to click the “New to District” checkbox when registering your student.
