For grades K - 12th, you must schedule an appointment to register your child(ren) at the Board of Education. Please call (937)278-5885 to schedule the appointment.
Required Documentation for Registration
- Completed Registration Packet - On-line Packet Available Below
- Original Birth Certificate
- Updated Immunization Records (All shots MUST be up-to-date or your child
CANNOT begin classes.) - Parent/Guardian Photo Identification
- 2 Proofs of Residency (MUST HAVE 2, NO EXCEPTIONS) - Acceptable proofs include: lease or deed, current DP&L bill, current Vectren bill, current cable/satellite bill, a stamped/sealed notarized residency affidavit along with 2 acceptable proofs of homeowner's or landowner's residency
- Custody Papers (If applicable) with Time Stamp
- Students in grades 10th through 12th must provide transcript.
